The PageRanks you see are approximate internal PageRanks, not Toolbar PageRanks. Look at the screenshots on the download page. Using the tool, I temporarily shifted PageRank to a recent blog post. Notice how the "highest PageRank URL" reported inside Webmaster Tools reflects that change. Yeah, its that easy.
