PPC Hero has details about a new Google AdWords beta named contact form extensions. Contact form extensions provides a contact form directly in the search ad, which a searcher can fill out and the advertiser can then use in the future to contact that lead. It is very similar to a lead acquisition form, but this one is found directly in an expanded Google AdWords ad.

PPC Hero posted an image that sums up how the form looks:

After the user fills out the form, Google will send you an email with a “lead ID” and other requested information. Google will give you a Google phone number to call, you then enter in the lead ID and Google will redirect the call to the phone number left by the customer.

The special ads only will show up in the number one position, with a plus sign. Advertisers will pay a maximum CPC for each lead submitted. These are only phone number based leads, not email or live chat based lead – although that may come.
